ELIGIBILITY REQUIREMENTS
Individuals who cannot read or use conventional printed materials as a result of physical limitations, i.e. blind, visually limited, physically limited, cannot hold a book or turn the pages, and reading/learning disabled are by law eligible. This also includes persons in private homes, nursing homes, hospitals, schools and other institutions.
To qualify for participation in the library service, the individual should secure a brief statement describing the specific disability characteristics. This should be written by a competent authority such as a doctor, ophthalmologist, registered nurse, professional staff member of a hospital or other institution or agency. In absence of any of these, a professional librarian can certify a patron. This statement should then be submitted to the Library.
In case of reading disability from organic dysfunction, "competent authority" is defined as a doctor of medicine or a doctor of osteopathy who may consult with colleagues in associated disciplines.
